Th Fss Implements Efficient Kiosks For Id Card Renewals
Patron or guardian may carry the ID card when using the booth. If the sponsor does not sign Form 1172-2, the sponsor must sign the form when picking up the card. If the sponsor is not present, the dependent will need the current attorney's permission to sign when picking up the card. If the DD 1172-2 is signed (digital signature for CAC holders or notarized water signature) and verified at the kiosk or uploaded to DEER in advance, the dependent may pick up the ID card without a sponsor. Sponsors are always allowed to register on behalf of their dependents.

Yes, the earliest you can apply for an ID card is 30 days at the kiosk and/or when the member receives a Medicare A or B card that can be loaded at the kiosk.
Only sponsors with a CAC can pre-submit a DD Form 1172-2 with DEERS Online Self-Service (this link requires CAC computer support). All sponsors must download the 1172-2 and sign it in front of a notary before checking out at the booth. The UPS Store in BX offers notary services for a low fee.
